Privacy Policy

As a current customer of Consolidated Insurance Center, Inc. dba Consolidated Insurance + Risk Management, hereinafter referred to as Consolidated, we would like to take this opportunity to both thank you for your business and to share with you the importance our agency places upon protecting the privacy of information we gather from you in accordance with applicable state and federal laws.

Consolidated is a member of the financial services industry and is subject to federal and state privacy laws regarding the collection and exchange of your information, such as HIPPA (Health Insurance Portability and Accountability Act), Gramm-Leach-Bliley, and the Fair Credit Reporting Act. Our policies and procedures comply with all federal and state privacy laws and regulations.

Information we collect and the means used to collect:

To provide those services usual and customary to service our customers, former customers and potential customers; we may collect (from you or from outside sources) and disclose (to companies we represent and their affiliates) the following types of information:

  • Application information which may contain your name, address, social security number, driver’s license number, date of birth, email addresses, marital status, family member information, and healthcare information.
  • Transaction information (used to service your account) such as claims and payment history, policy coverages, and premiums.
  • Consumer-reporting agency information such as driving record, insurance score report, credit, medical, and financial information.
  • Compliance with requests from regulatory and law enforcement authorities.

To collect customer information from the above-stated sources, Consolidated may use the following means of communication to gather information: written, in-person, telephone, facsimile, electronically, and/or on-line. Please note, our privacy practices continue to apply to your information even if you cease to be a Consolidated customer.

Nonpublic Personal Information and Nonaffiliated Third Parties:

We do not disclose nonpublic personal information about you to nonaffiliated third parties, except under certain circumstances where we are permitted by law to disclose nonpublic information we have collected about you. For example, we may disclose nonpublic personal information about you to nonaffiliated third parties in order to offer or support a product or service that you have requested or that you already have with us, such as individual or business insurance policies.

Our practice regarding information confidentiality and security:

We restrict access to nonpublic personal information about you to those employees who need access to that information in order to provide products and services to you. We maintain physical, electronic and procedural safeguards to guard your information. Consolidated appreciates your business. In order to continue building upon that relationship, we believe it is necessary - not only from a legal standpoint, but also as a sound business practice - that our customers understand the care our agency uses in handling your information. Consolidated will continue to monitor the effectiveness of this privacy policy.

We are required by law to send this Privacy Policy Notice. Be assured that our policy regarding information we receive about you remains unchanged. We will not share any such information except in the servicing of your insurance/benefits program.
